How do I register to become a new patient?
Register in our secure online portal. To register a child from birth thru 11 years old, click this link: Pediatric Patient Registration. For teens and adults (12+ yo), click this link: Teen & Adult Patient Registration. Make sure to upload a copy of your driver’s license, health insurance card, and pharmacy card during registration. If you or your child has Medicaid or ARKids 1st (part A or B), Dr. Shelly Bray must be listed as the primary care physician (PCP) before the visit. If Dr. Bray is not listed as the PCP, complete the Medicaid PCP Change Form by clicking this link: Medicaid PCP Change Form Online and then return to this page to click the age appropriate registration link.

How do I schedule an appointment?
Call (870) 464 – 1515 to speak to an operator. Be prepared to provide your full name, date of birth, current address, phone number, and insurance information. You will want to let the operator know about all the concerns you would like addressed so that we can schedule the appropriate amount of time to care for your needs. If you have a complicated medical history, we recommend completing a Medical Records Request Form before your first visit, if possible. You may email your completed medical release form to [email protected] and we will request your records from your previous primary care provider.

What should I bring to my first appointment?
- A valid government issued driver’s license or other photo identification.
- Your health insurance card and your pharmacy benefit card, if applicable.
- Your copay (if you have one) or $75 if your plan has a deductible that has not yet been met.
- All of the medications you are currently taking (prescription & over-the-counter).
- Any previous medical records you may have including immunizations.
